Meeting Your Immunization Requirement 101

THE REQUIREMENT

For EVERYONE:

  • 2 doses of measles/rubella vaccine (MR or MMR); or
  • Immunity to measles and rubella by blood titers

For those 18 & Younger (in addition to the MMR):

  • 3 doses of Hepatitis B vaccine; or
  • Immunity to Hepatitis B by blood titers

WHAT IF I NEED THE VACCINE OR I CAN'T FIND MY RECORDS?

Get the immunization at:
  • Your family physician office
  • Local medical clinic
  • Your county Public Health Department
  • MMR $45.00 and Hep B $12.00-$25.00 at the Student Health Center

HOW DO I MEET THE REQUIREMENT?

Submit a photocopy of any following to the STUDENT HEALTH CENTER:
  1. Childhood immunization record (yellow card);
  2. High school transcript IF immunization information is documented on it;
  3. Lab reports from your doctor showing immunity (titers)
